Sage Intacct for Healthcare Organizations: Setup, Dimensions and Reporting

Last updated: October 8, 2026 · Independent guide · We do not sell rankings or coverage.
Setting up Sage Intacct for healthcare means building four things in the right order: an entity for each legal entity (the management company and each professional corporation), clinic locations under those entities, dimensions for service line, provider and payer, and statistical accounts that hold visit counts from your practice management (PM) or EHR system. Once those are in place, you can load monthly PM summaries, allocate shared costs by visits and report cost per visit by clinic and provider. Each step below uses menu paths from Sage’s help documentation, checked on October 8, 2026.
If you are still comparing systems, see our guide to healthcare accounting software.
Before you start
- A multi-entity company with a top level. Sage has been moving single-entity companies to this framework in phases since June 2022 (Sage Intacct help, October 1, 2026).
- An administrator account. Adding entities needs a business user with admin privileges, and Sage notes that adding an entity “can incur an extra charge” (Sage Intacct help, October 1, 2026).
- Platform Services for user-defined dimensions such as Provider and Payer. Sage says additional fees might apply for user-defined dimensions (Sage Intacct help, October 1, 2026).
- The Dynamic Allocations subscription to allocate shared costs by visits (Sage Intacct help, October 1, 2026).
- EMRConnect (optional), which also requires Platform Services and Web Services and is US-only (Sage Intacct 2025 Release 4 notes, no date shown).
- A signed business associate agreement if any patient names will reach the ledger. Sage states it “is willing to enter into a Business Associate Agreement with eligible healthcare clients” (Sage, no date shown). Sage publishes no prices for any of these, so ask for an itemized quote.
The structure this guide builds
The mapping below is our suggested design for a physician group or clinic network; adapt it to your own reporting.
| What you report on | Intacct object | Example values |
|---|---|---|
| Legal entity (MSO, each PC) | Entity | E100 Management Co, E200 Medical PC |
| Clinic or practice site | Location (child of entity) | North Clinic, Eastside Clinic |
| Service line | Department | Primary care, Imaging, Physical therapy |
| Rendering provider | User-defined dimension | One value per physician or clinician |
| Payer or payer class | User-defined dimension | Medicare, Medicaid, Commercial, Self-pay |
| Visits, encounters, wRVUs | Statistical accounts | Patient visits, New patients |
The standard Employee dimension works for providers only if every clinician has an employee record, which contracted and locum providers often do not.
Step 1: Add an entity for each legal entity
Go to Company > Setup > Configuration and select Add next to Entities. Enter an ID, name, federal tax ID, operating country and state tax ID, then save (Sage Intacct help, October 1, 2026).
The entity ID is permanent, and Sage suggests IDs such as E100 and E200. Sage describes an entity as “a separate tax identification or a separately secured, fully balancing set of books,” so do not create one for every clinic. With Consolidation, you also need one elimination entity per consolidation book currency.
Step 2: Add clinic locations under each entity
Go to Reports > Setup > Dimensions and select Add next to Locations. Enter an ID and name and choose the parent; Sage states that the entity is the ultimate parent location (Sage Intacct help, October 1, 2026).
IDs can contain only letters, numbers and underscores. If you have many sites, you can upload locations by CSV through Company > Setup > Import data.
Step 3: Enable inter-entity transactions
Management fees charged by the MSO to each professional corporation are inter-entity entries. Map due-to and due-from accounts at Company > Setup > Configuration > Inter-entity account settings > Inter-entity account mapping, then enable inter-entity transactions on the Multi-Entity Management page. New companies default to the Basic plan, which uses two accounts per entity; the Advanced plan uses up to four accounts per entity pair (Sage Intacct help, October 1, 2026).
A journal entry that spans entities then gets automatic offsetting entries. Only Location flows to them, so you balance other dimensions yourself.
Step 4: Set up service lines in Department
Add service lines as Department values: go to Reports > Setup > Dimensions and select Add next to Departments (Sage Intacct help, October 1, 2026). The General Ledger configuration’s dimension settings control which dimensions show on GL entries and which must balance (Sage Intacct help, October 1, 2026).
To use dimensions in Accounts Payable or other applications, go to Company > Admin > Subscriptions, select the application and choose Configure. A dimension used on a transaction cannot be deactivated, so settle the list before go-live.
Step 5: Create Provider and Payer as user-defined dimensions
Sage recommends creating user-defined dimensions from the General Ledger rather than from Platform Services. Go to General Ledger > Setup > Configuration, open Advanced GL options, find User-defined dimensions and select Add. Enter the name (Provider) and deploy it into a custom application (Sage Intacct help, October 1, 2026).
Grant access at Company > Roles under Additional dimensions permissions, then add values at Reports > Setup > Dimensions by selecting Add beside the new dimension (Sage Intacct help, October 1, 2026). Repeat for Payer. Avoid names containing “event” or “log,” which Sage says can break integrations.
Use the provider’s NPI or PM provider code as the value ID so the monthly import matches directly.
Step 6: Link providers to their home clinic
A dimension relationship can autofill one value from another, for example the Location from the Provider. Go to Platform Services > All > Objects, select Edit beside the base dimension, and define the relationship (Sage Intacct help, October 1, 2026).
CSV imports and API calls do not autofill, so your import file must still carry Location on every line. Sage also says relationships assist coding rather than control it, and points to Smart rules for enforcement. Leave providers who work across clinics unlinked.
Step 7: Create statistical accounts for visits
Go to General Ledger > All > Accounts and select Add beside Statistical accounts. Use a separate number range, and under Require dimensions select Location, Department and Provider (Sage Intacct help, October 1, 2026).
Then create a statistical journal at General Ledger > Setup by selecting Add next to Statistical (Sage Intacct help, October 1, 2026). Finally, add a statistical account group at General Ledger > All > Financial report structures, selecting Add beside Account groups and setting the structure type to Statistical accounts. Sage states that accounts must be rolled into account groups before they appear on financial reports (Sage Intacct help, October 1, 2026).
Step 8: Load monthly summaries from the PM or EHR system
Post summaries, not claims. The figures below are an illustration of one clinic’s monthly summary.
| Line | Account | Dimensions | Amount (illustrative) |
|---|---|---|---|
| 1 | Gross patient charges (credit) | North Clinic, Primary care, Provider 1001, Commercial | $120,000 |
| 2 | Contractual adjustments (debit) | Same | $48,000 |
| 3 | Patient accounts receivable (debit) | North Clinic | $72,000 |
| 4 | Patient visits (statistical) | North Clinic, Primary care, Provider 1001 | 640 visits |
The first way to load it is a CSV upload through Company > Setup > Configuration > Import data, using the GL journal entries template for lines 1 to 3 and the statistical journal entries template for line 4. Download a fresh template each time, because Sage updates templates between releases (Sage Intacct help, October 1, 2026; Sage Intacct help, October 1, 2026).
The second is the Web Services API, whose statistical journal entry create function accepts Location, Department and other dimension IDs, plus custom fields, on each line (Sage Intacct Developer, no date shown). The third is EMRConnect, covered in Step 10.
Step 9: Allocate shared costs by visits
Subscribe at Company > Admin > Subscriptions > Dynamic Allocations > Subscribe and create a user-defined book and journal for allocation entries. Then go to General Ledger > Setup and select Add beside Account allocation definitions (Sage Intacct help, October 1, 2026).
Set the source pool to your billing office or MSO overhead account group, and choose the Dynamic – relative account statistical method with your visits account group as the basis. Set Accumulation to Activity so the split uses visits recorded in the basis period. One allocation can generate up to 5,000 lines.
Step 10: Connect EMRConnect if you subscribe
EMRConnect is Sage’s own import tool for healthcare. Sage says it “works with most delimited flat files that are exported from EMR” systems, and lets you set up field mappings for as many formats as you need (Sage Intacct 2025 Release 1 notes, no date shown). You can create your own value transformations on the Value Transformations page by selecting New EMR Value Transformation (Sage Intacct 2025 Release 4 notes, no date shown).
Subscribers whose company was created from the General or Nonprofit QuickStart template can install the EMR Ambulatory Care Insights dashboard from Dashboards > All > Dashboards under Dashboards library, which tracks patient encounters, clinician efficiency and location performance (Sage Intacct 2025 Release 2 notes, no date shown).
Common mistakes and how to fix them
- One entity per clinic. This multiplies inter-entity entries and entity charges. Make clinics that share a tax ID locations instead.
- Visit counts missing from reports. Add the statistical account to a statistical account group.
- Imported lines with no clinic. Relationships do not autofill on imports. Add Location to the export and make it a required dimension.
- Allocation produces odd splits. Visits were loaded late for the period. Post visits first, then rerun and check the allocation log.
- Patient detail in the ledger. Post summaries only. Where names arrive, such as on patient refunds, have the BAA in place.
Limitations of the native features
Intacct does not build the PM extract for you. Without EMRConnect, someone produces a summary file each month, and Sage does not name the EMR vendors EMRConnect supports. Inter-entity entries carry only Location. User-defined dimensions, extra entities and Dynamic Allocations can all add unpublished cost.
When organizations add a third-party tool
Groups add other tools when several PM systems feed one ledger after acquisitions (an integration platform or partner connector on the Intacct API), when providers are paid on wRVUs or collections (a compensation tool that posts results), or when budgets need visit-driven forecasting. If you are still weighing the ledger itself, the alternatives in our multi-entity ERP guide handle entities and consolidation differently, and our EMR and practice management systems guide covers the systems you will be importing from.
How we researched this
We read the pages that rank for “sage intacct healthcare”: Sage’s healthcare page, implementation partner pages from BCS ProSoft and BPM, and an ERP Research profile. None gave menu paths or covered statistical accounts or visit-based allocations. Each step comes from Sage Intacct’s help center and release notes, checked on October 8, 2026. Our wider view on hosting choices is in cloud versus on-premise accounting for regulated industries.

Frequently asked questions
Should each clinic be a separate entity in Sage Intacct?
Only if the clinic is a separate legal entity with its own tax ID. Otherwise make it a location under the professional corporation that operates it.
How do I track patient visits in Sage Intacct?
Create a statistical account for visits, a statistical journal and a statistical account group. Load visit counts each month from your PM system with the same Location, Department and Provider values as your revenue.
Can Sage Intacct connect to my EHR or practice management system?
Yes, through CSV imports, the Web Services API or Sage’s EMRConnect subscription. Sage does not publish a list of EMR vendors EMRConnect supports, so ask for a test with your own export file.
Will Sage sign a HIPAA business associate agreement?
Sage states it is willing to sign a BAA with eligible healthcare clients. HHS guidance treats a cloud provider that stores electronic protected health information as a business associate, even if the data is encrypted.
